Promedic Hospice team develops a Plan of Care that is tailored to each patient’s needs in addition to those of the family who provide supportive care. The team usually consists of the following individuals:
The patient’s personal physician
Hospice physician (or medical director)
Registered Nurses

Hospice Medical Social Workers
Hospice Chaplains
Registered Dietitian
